The survey will ask you what institution you study at, and whether or not you chose to be tested at University, and to state the reason why you made that choice.

It will also ask you to provide feedback on how you found out about the testing, how the overall experience could be improved for you, and what else you’d have like to have known about the testing prior to making a decision.

Finally, it will ask you some demographic information. The survey should take no longer than a few minutes to complete and it is anonymous.
