SWAI Evidence Policy

Your evidence can be:

• A letter or report from medical professional e.g. doctor, paediatrician, consultant, clinical psychologist signed and on letter headed paper.
• The Disabled Students Allowance Disability Evidence Form.  You can give this to your medical professional to complete (please note this is only available to SFE funded students only).
• Evidence from school or college of your educational needs, for example a statement of SEN or an IIP.

 Your evidence needs to include:

• Confirmation of an autistic spectrum condition.
• Outline of effects.
• Date of identification.
• Impact on day-to-day activities.
• Potential effects on study and social interactions.

Your evidence must also include an explanation of how you have previously been supported in an educational environment or how your capacity to learn is affected by your autism.

As different countries can have vastly diagnostic testing and reporting techniques, SWAI will consider these reports on a case-by-case basis to determine appropriateness for study in Higher Education.

For applicants and students requiring physical adaptions to bathrooms, bedroom and kitchen design / function:

• An Occupational Therapist is required to submit a detailed report to the University.
• The report should be emailed to StudentInclusion@lboro.ac.uk where a Disability Advisor will acknowledge receipt and work with you, your OT and the Student Accommodation Centre to consider the content of the report.
• It can be expected that site visits will be required at various stages by key stakeholders to assist with the process and to help ensure adaptions are made in time for the start of term. 
• You must bear this in mind in order to provide the final assessment to us by 31st May.

The following should be included in your report:

1. Name and contact details of the Occupational Therapist
2. Date and location of assessment
3. Student’s name and date of birth
4. Student’s diagnosis
5. Those present at the assessment
6. Current mobility and handling situation for:
   1. Stairs
   2. Outdoor mobility
   3. Transfers: bed, chair, shower, toilet
   4. Any other relevant domain
7. Daily living skills:
   1. Communication
   2. Eating and drinking
   3. Washing and dressing
   4. Household tasks
   5. Leisure
   6. Any other relevant skills
8. A room plan of the current room layout and the recommended new room layout required, including heights of required equipment.
9. Recommendations for the provision of adaptions in:
   1. Bathroom
   2. Kitchen
   3. Bedroom
   4. Indoor/outdoor mobility
10. Recommendations must include:
    1. Item description and detail
    2. Supplier details
    3. Website link to the item
    4. Item cost

If you use a wheelchair / mobility aids and require adaptions to be made to the design and function of your bedroom and/or bathroom and/or kitchen, you will need to supply an Occupational Therapy report. Further information about this report is contained in the physical disability tab above.

• The Disabled Students Allowance Disability Evidence Form. You can give this to your medical professional to complete (please note this is only available to SFE funded students only).

Your evidence needs to include:

• Confirmation of a working diagnosis.
• Where possible, an outline of symptoms and likely impact on day-to-day activities at University.

A full diagnostic assessment (which means it must adhere to, and include content as outlined by, the SpLD Working Group/DfES Guidelines, as defined by the SpLD Assessment Standards Committee completed by either:

• A Chartered Educational Psychologist (EP) holding a qualification recognised under the SpLD Working Group/DfES Guidelines and registered with the Health and Care Professions Council
• A Specialist Teacher Assessor (STA) holding a qualification recognised under the SpLD Working Group/DfES Guidelines and a valid Assessment Practising Certificate issued by either The Dyslexia Guild,  the Professional Association of Teachers of Students with Specific Learning Difficulties, or the British Dyslexia Association

Form 8 or Access Statements from your compulsory education institution are not sufficient to access support at Loughborough University as they do not meet the above criteria. Form 8 and Access Statements cannot be accepted.

All applicants are encouraged to secure a full diagnostic assessment before starting their studies, rather than wait until their arrival on campus. This allows your support to be available from the start of the course, and for your assessments/exams to be taken with the appropriate support in place e.g. 25% extra time.

An assessment by a medical or educational professional e.g. physiotherapist, occupational therapist, consultant paediatrician.

A diagnostic report by either a Chartered Clinical Psychologist or a Psychiatrist. However Chartered Educational Psychologists and specialist teachers with the relevant training may identify patterns of behaviour that would strongly suggest ADHD for the purpose of HE support.
