CDS Accommodation Requirement form
Please use this form to advise the University about any specific accommodation requirements you have in relation to a disability/long term health condition.

The University supports disabled students as defined under the Equality Act (2010): a physical or mental impairment which has lasted, or is likely to last, for more than 12 months and which has a substantial and adverse effect on an individual’s ability to carry out day-to-day activities (including education).
So that we can identify, allocate or adapt suitable accommodation in sufficient time, please complete this form as soon as possible. At the end of the form you will be asked to upload evidence of your disability. A list of acceptable evidence can be found online here (we would prefer you upload your evidence rather than send it in at a later date). The disability and/or mental health support team will review your form alongside the medical evidence and, if accepted, will email an Accommodation Profile to the Student Accommodation Centre (SAC). This document advises SAC of your requirement.
Please be assured that this form is not used as part of admission decisions and is used solely to help ensure we do all we can to allocate you the most appropriate accommodation. If you have not already done so we also recommend you register with the disability and/or mental health support team here in order to access support after your arrival into Halls.
